-Pet Import Information Sheet (Pet documents #1-5 and #8) 

-Pet Reservation Request Form (Pet document #6)

-AMC Risk Acknowledgment Document (If applicable) (Pet document #7)

Work with your DTS point of contact to get the same flight as your dependents back to Bahrain.
